Here's a true example (names are changed).

Jack got mad when Joe was in emergency room.

1. Why?
The church people didn't show up quickly.

2. Why?
People didn't know about it.

3. Why?
Lack of communication.

4. Why?
No prayer chain in place.

5. Why?
No one took the initiative to do it.

The problem is now solved.

Next time you see a problem, try applying the 5 whys and see if it helps.
