The competition version of quake 3 is still the best first person shooter for deathmatch. There are still competitions for this game.
When quake first came out I used to make levels and design things for it. The thing is that the level editors were rediculously difficult to use. Autocad is much easier.

I do agree that its the best game out there though i prefer freeze tag over death match
But i have to say that i have never tried to make any levels though.
