He is probably referring to Adam and Eve's command to multiply and fill the Earth. Cain and Able maybe Seth would have had to marry sisters for this to happen. At least one man would have had to marry a sister. After that the second man could have married a niece.
Yes, all of the first generations were from close familial relationships. But, where there is no law, sin is not accounted. The law against close familial relationships didn't come about for some 2,000 years after the creation event.
